About the Role:

CBH L&D facilitators are responsible for developing the knowledge and skills of our Cityfolx workforce. They facilitate the organization's learning strategy to include but not limited to Orientation , Market Academy 1, Market Academy 2 and other educational initiatives. They ensure that our supporting and care teams can meet its current and future business objectives and deliver a high quality Cityblock Health Care  model and services that meet our member's needs. 

Requirements for the Role:

  • You have a bachelor’s degree in education, psychology, public health, sociology or a related field.
  • Field-based work experience for a similar population served by Cityblock
  • You are an independent self-starter, a leader, and a strategic thinker who is excited about the big picture of whole community health, and the ongoing evaluation and iteration of our care model
  • You have some experience training individuals and groups
  • You are charismatic, engaging, dynamic, creative, and accessible
  • You have solid presentation and facilitation skills
  • You have the ability to work effectively in a fast-paced  environment
  • You have the desire to contribute to a learning and evolving model of care, and a quickly growing staff
  • You are excited about how technology can support your work and help drive the ongoing evaluation toward new and better care.
  • You have the ability to travel periodically to support implementation activities

How We Define Success:

  • Lead or co-lead orientation sessions for new team members on topics such as Cityblock Values, Tools & Technology, Cityblock Care Model, Navigating Commons, & Documentation/Care Planning. 
  • Support operational tasks associated with planning for orientation. 
  • Serve as a true model of Cityblock's culture for all new hires, at all levels, joining the company.
  • Engage onboarding managers to ensure that they have strong onboarding plans for their new hires. 
  • Work closely with market ops to ensure that in market onboarding training (market academy 2) is running smoothly. 
  • Develop and maintain post-training surveys
  • Work closely with other trainers at Cityblock to ensure an optimal orientation experience for new hires and a culture of learning for all care team members
  • Lead deep dive sessions post-orientation for new team members as needed.
  • Audit team results, identify gaps in training and implement improvements in training programs.
  • Partner with implementation to lead training sessions in new markets as we expand. 
  • Execute with strong project management skills, including planning and logistics, monitoring work-plans and managing against milestones, knowing what to prioritize and why, knowing who to keep informed, and when to escalate.
  • Embody Cityblock’s ‘ways of being.’  Lead with excellence and foster high-performing teams.
  • Develop team members through advocacy, coaching, mentoring and training.
  • Provide inputs to Cityblock’s training strategy and specific training development plans.
  • Assess training effectiveness to ensure classroom skills and techniques are incorporated into team members’ work

Nice to Have, But Not Required:

  • You have a bachelor’s degree in education, psychology, public health, sociology or a related field.
  • Experience training or developing curriculum for community health workers or care coordination staff
  • You have familiarity with traditional and modern training methods, tools and techniques
  • Experience in a technology-enabled healthcare setting
  • Knowledge of instructional design theory and learning principles
  • Multi-lingual
