About Call9: Call9 is breaking the mold of traditional Emergency Medicine and telemedicine while reinventing 911. Our patient-centered proprietary telemedicine platform connects off-site emergency physicians with on-site emergency medicine trained Clinical Care Specialists (CCS). Call9 provides a spectrum of emergency care services at the patient’s bedside in skilled nursing facilities.
Position Summary:The Intake Assessment RN is responsible for reviewing the assessments completed by high-risk patients admitted into our Skilled Nursing Facilities.
Responsibilities
- Facilitate and complete the intake assessments of high-risk patients admitted into the Skilled Nursing Facilities
- Review all pre-enrollment documentations, including Call9/Facility enrollment documentation, utilizing consistency tools to ensure accuracy and appropriateness for care
- Complete regular reports and updates for the Clinical Operations team to review
- Provide Clinical Operations team with feedback on documentation errors while striving towards documentation improvement
- Uses clinical judgement to make assessments on previous medical records to ensure accuracy
- Additional assignments and duties as needed
Qualifications
- Graduate of a 2-4 year accredited nursing program
- NYS RN License required
- Knowledge with insurances, ISNP’s and critical care preferred
- Experience using EMR, preferred
- Problem-solving skills and ability to do investigations on medical history
Education
- BSN preferred
- Graduate of accredited nursing program
- Valid and active NYS RN License required
