About the Role:

Mobile Integrated Care (MIC) is a team that provides essential and high value urgent and acute care services to our growing number of Markets, who are accountable to our members and their health outcomes. We currently offer three services lines:

  • Urgent care: on-demand response for members experiencing an acute event, either in-person via our community paramedicine program, or virtually with support of a triage nurse or emergency provider.  
  • EMT-assisted provider visits: a clinical visit program that pairs a virtual provider with an in-person EMT, allowing our members to see a provider via video call wherever they feel most comfortable.
  • Virtual urgent care: building 24/7 coverage for video visits with prescribing providers

This is only the beginning. MIC is a key part of Cityblock’s strategy to help our members stay out of the hospital, and the potential growth in this team is practically limitless. We want to grow; the volume of members we’re serving, the number of geographies we’re operating in, our operating hours, and the types of services lines we’re providing to our Markets. By the end of the year we hope to be in 5+ states, serving 50,000+ members. 

We are seeking a rockstar operations and people director to strengthen, enhance, and expand our existing urgent care and community paramedicine programs to provide acute-level care to our members in the comfort of their homes, in lieu of an inpatient setting.

Primary Responsibilities

As the Director of Mobile Integrated Care, you will direct the operational success and scalability of our major programs.

  • Supporting the existing teams in continuing to hit their SLAs.
  • Working both within Urgent Care and with the markets to rapidly increase (acuity-appropriate) demand for Urgent Care services
  • Transform EMT-Assisted Clinical Visits from MVP to a scalable product with standardized processes across markets
  • Identify opportunities to streamline operations, making it easier for internal stakeholders and members to utilize these services.
  • Identifying the optimal org structure, skill sets and team mix
    • Hiring and building into this vision
  • Helping these teams grow into additional markets and increase their volume
  • Hire and build the team to support this plan
  • Defining KPIs for these service lines, with testable hypotheses on the outcomes that you expect to see (drive down total cost of care, improve member outcomes, drive member satisfaction, etc).
  • Partnering with the Head of Acute and Episodic Care to identify members who qualify for this service.
  • In partnership with Market General Managers, funneling members into this service line in order to grow it across markets and test your hypotheses, with a goal of growing across Cityblock’s major markets in NY, CT, MA, and DC.
  • You will then build on this foundation to launch incrementally expanded services and explore broader market opportunities.

Requirements for the Role:

  • You have a bachelor’s degree and 7+ years of experience managing diverse operations teams that serve multiple functions, including clinical and non-clinical roles.
  • You have deep experience and a deep comfort working within a high-paced, ambiguous, ever-evolving startup environment  
  • People management experience, ideally within a growing team.
  • Have excellent project/process management skills (particularly with modern tool suites built off of cloud-based collaboration), and you are rigorous, energetic, agile, flexible, and aren’t afraid to do the work yourself
  • Proven ability to effectively lead cross-functional teams in building processes that deliver results.
  • Demonstrated experience in care coordination, clinical standards, and measuring clinical outcomes
  • Strong “build” capabilities - able to define an MVP, build a team to execute it, and launch the operations to facilitate their success.
  • Detail-oriented with strong presentation skills.
  • Exceptional interpersonal skills and judgment. Ability to maintain excellent working relationships across departments, understanding the need to bring in key team members at the appropriate time during the implementation life cycle.
  • You are compassionate, open-minded, and non-judgemental
  • You have the ability to think quickly and solve problems in the moment
  • You are excited to grow your leadership skills and have demonstrated this through prior experience in development of a program.

You will: 

  • Work in a radically different model of healthcare, leveraging expanded care teams, in-person and virtual care models, as well as a proprietary homegrown IT platform for collaborative patient assessment and management 
  • Expect collaboration, shared-decision making, and partnership across clinical and non-clinical care team members.
  • Be centered in our Virtual Hub; supporting members across all of our markets and with various needs.
  • Identify and collaborate closely with existing outpatient behavioral health providers, primary care providers and specialists to promote shared decision making in our member’s health and successfully co-manage (WRAP) members
  • Be responsible for defining and scoping the roles that need to be filled within your team, and partnering with Talent to interview and hire the best candidates.

  • Manage a complex population of members in a collaborative, interdisciplinary team model 
  • Meet monthly and quarterly targets for visits with Cityblock members 
  • Engage in target setting for new clinical initiatives and managing those targets 
  • Active participation in practice development, redesign, and innovation 
  • Effectively lead operations for the Mobile Integrated Care team
  • Hospital visit diversion
  • Define success metrics for any new partnerships


Nice to Have, But Not Required:

  • Experience managing a distributed team.
  • MBA or equivalent 
  • Experience within a fast-growing startup environment 
  • Interest or experience in analysis of healthcare data
