Cityblock Health is a new type of healthcare company, operating out of Brooklyn and backed by Alphabet’s Sidewalk Labs, along with some of the top healthcare investors in the country.

Our mission is to radically improve the health of urban communities, one block at a time. Importantly, our solutions are designed specifically for Medicaid and lower-income Medicare beneficiaries, and we bring the capability to deliver care in the home and neighborhood with our field-based teams.

In close collaboration with community based organizations and a leading commercial partner, we are reorganizing the health system to focus on what matters to our members. We deliver personalized primary care, behavioral health, and social services through a network of neighborhood hubs with deep community-based partnerships and world-class technology.

We partner with payers and at-risk providers, taking capitated financial risk to care for their most vulnerable, high and rising risk members. Additionally, we invest in strategic partnerships with community-based and social services organizations, in some instances bringing them into the value equation through sub-capitation and performance-based compensation.

Over the next year we’ll grow quickly, including entering new markets, each with their own commercial relationships and field-based teams. This role will be a key contributor to the success of our business.

The role:

In this role, you will implement and build upon an outreach strategy aimed at engaging members who face economic and social challenges in addition to having mental and physical health needs. The work of our outreach team establishes trust with our members and builds the foundation for our care model. Your work will be largely field-based, supporting a team who will be meeting members in the community.

As an Outreach Lead, you will be oversee a team of outreach specialists who are responsible for locating and engaging with members assigned to Cityblock. This includes coaching and supporting the outreach specialists and community health partners (CHPs) on Cityblock’s outreach strategy, as well as performing outreach to your own panel of members.

You will:

  • Supervise a team of 5-8 outreach specialists, each having a panel of ~100 members to engage
  • Organize and manage community outreach events 
  • Collaborate with the Community Health Partner (CHP) Leads on outreach and engagement initiatives involving CHPs
  • Manage and track all aspects of outreach activity, including outreach performed by other care team members, to insure best practices are followed and performance goals are met
  • Develop and iterate on best practices for engaging ‘difficult to reach’ members
  • Utilize data obtained from outreach activity in Commons, including success rate by time of day, geographic area, and other variables, to optimize our outreach strategy
  • Provide training to new hires, as well as ongoing in-services to all team members including daily planning and logistics, as well as documentation in Commons
  • Accompany other team members on outreach visits to provide in-person support and guidance
  • You will be accountable for the expansion of Cityblock’s member population:
    • Engage and foster trust quickly with members, acting as the face of Cityblock and the key point-of-contact to assist members prior to full enrollment.
    • Meet with members in their homes, neighborhoods, and the Cityblock health hub; nearly all of your work will be done in the field.
    • Participate in case conferences for members you have engaged to recommend an appropriate team and interventions.
    • Go above and beyond to connect with patients and partners in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day.
    • Document all outreach, intake, and assessment activity with in Commons in a timely fashion

You’d be a good fit if:

  • You have experience with community outreach, especially with difficult to reach individuals
  • You have experience walking door-to-door, with public speaking, or previous sales experience and training 
  • You have experience managing a team and overseeing day-to-day personnel needs
  • You excel at empathy and human interactions, and want to improve the health of individuals and whole communities.
  • You are an independent self-starter and a strategic thinker who is eager to impart your experience on a new team of outreach specialists
  • You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care.
  • You take accountability for achieving performance targets and motivating a team to achieve these goals

Nice to have:

  • Experience working with individuals with mental health and substance use diagnoses.
  • Training in motivational interviewing, behavioral activation therapy, or problem solving treatment.
  • Direct familiarity with clinical settings.
  • Multilingual
