The role:

In this role, you will be a part of the first team providing direct services to members, who face economic and social challenges in addition to having mental and physical health needs, through our innovative care model. You will be associated with our first clinical “hub,” a primary care practice that in many respects will look more like a community center than a doctor’s office. You work will be mobile, field-based in the community. It is integral to our care model that we meet people where they are, both physically and emotionally.

As an Outreach Specialist, you will be responsible for locating and connecting with members that we believe would benefit from the Cityblock care model, and engaging with them around their needs. This initial conversation or series of conversations with members will begin the intake process, and will be the stepping-off point from which a member is connected to a Community Health Partner to develop a long-term, trusting relationship. Until this connection is fully made, you will be a member’s point person, available and accessible to assist with whatever may be needed, to troubleshoot, to educate, and to be a listening ear. You will work alongside, and will be supported by, a care team associated with each member.

 

You will:

  • Provide telephonic, written, digital and face to face outreach and engagement services for each eligible member. This will also include group welcome events and community outreach events.
  • Use available data to learn more about a potential member and inform the outreach process. 
  • Complete the intake and consent process, and provide a handoff to the Community Health Partner or other care team lead upon full enrollment into Cityblock Health.
  • Engage and foster trust quickly with members, acting as the face of Cityblock and the key point-of-contact to assist members prior to full enrollment.
  • Meet with members in their homes, neighborhoods, and the Cityblock health hub; nearly all of your work will be done in the field.
  • Participate in case conferences for members you have engaged to recommend an appropriate team and interventions.
  • Go above and beyond to connect with patients and partners in a non-judgmental, respectful and empathic manner, to meet their needs, and to provide feedback to the system as a whole as we strive to do better every day. 
  • Work daily within our custom-built care facilitation platform, which will enable you to collect data, organize information, track tasks, and communicate with staff, patients, and family members. This platform is being built for a mobile workforce, and you will be among the first cohort to field-test our technology, provide feedback to the product development team, and, over time, become part of a super-user group to assist in onboarding and supporting others.

You’d be a good fit if:

  • You have experience with patient navigation, management, or any kind of direct service provision.
  • You have a passion for working within the community you are a part of, or have been a part of in the past.
  • You excel at empathy and human interactions, and want to improve the health of individuals and whole communities
  • You are an independent self-starter and a strategic thinker who is eager to learn, improve, and grow.
  • You are excited about how technology can support your work and help drive the ongoing evaluation toward new, better, care.

Nice to have:

  • Experience working with individuals with mental health and substance use diagnoses.
  • Training in motivational interviewing, behavioral activation therapy, or problem solving treatment.
  • Direct familiarity with clinical settings.
  • Experience with community outreach or community organizing.
  • Multilingual
