Who we are

Cityblock Health is a new type of healthcare company, operating out of Brooklyn and backed by Alphabet’s Sidewalk Labs, along with some of the top healthcare investors in the country.

 

Our mission is to radically improve the health of urban communities, one block at a time. Importantly, our solutions are designed specifically for Medicaid and lower-income Medicare beneficiaries, and we bring the capability to deliver care in the home and neighborhood with our field-based teams.

 

In close collaboration with community based organizations and a leading commercial partner, we are reorganizing the health system to focus on what matters to our members. We deliver personalized primary care, behavioral health, and social services through a network of neighborhood hubs with deep community-based partnerships and world-class technology. 

 

We partner with payers and at-risk providers, taking capitated financial risk to care for their most vulnerable, high and rising risk members. Additionally, we invest in strategic partnerships with community-based and social services organizations, in some instances bringing them into the value equation through sub-capitation and performance-based compensation. 

 

Over the next year we’ll grow quickly, including entering new markets, each with their own commercial relationships and field-based teams. This role will be a key contributor to the success of our business.

 

The Role:

We are seeking an innovative, execution-focused leader who will drive the conceptualization, development, and implementation of a portfolio of clinical, operational, and technical/digital capabilities that advance Cityblock’s care model and operational performance in ways that enable us to achieve superior clinical outcomes, quality results and member experience. As a leader of Cityblock’s Innovation and Execution Group, this exceptional person will instill innovation, disciplined capability development, and continuous performance improvement as a core feature of Cityblock’s culture and a source of competitive advantage.

This leader will identify promising new capabilities for Cityblock and creatively develop bespoke capabilities entirely new to healthcare delivery. This leader must deeply understand the Cityblock care delivery model and operating model, and align with Clinical, Operations and Product leadership to diagnose and identify the most important capability gaps and issues that need to be addressed through innovative programs and performance improvement initiatives.

This leader will serve as the internal steward of a disciplined innovation management approach and will own a portfolio of strategic initiatives that Cityblock elects to pursue through evaluating and prioritizing potential new innovations, considering capabilities needs, investment priorities, risks/benefits, and other relevant trade offs. For initiatives prioritized for development, this leader will guide teams to develop and rapidly test pilots that then graduate into full scale implementations across Cityblock. This responsibility includes end-to-end program management and, over time, optimizing new capabilities to reach their full potential as Cityblock scales and grows. All functions and local markets are in scope and must be continually evaluated to ensure that Cityblock can efficiently and effectively support high growth while maintaining a desired level of fidelity to our core model.

Illustrative examples of programs within this leader’s portfolio may include and are not limited to:

  • Remote patient monitoring and digital technologies deployed in the field
  • Streamlining of care team coordination processes, applications and tools
  • New tools and processes for collaborating with local social services and community organizations
  • Digital tools to enable clinicians, non-clinicians and clinical operations team members to communicate across health care settings
  • Clinic throughput, process efficiency, scheduling efficiency

You will:

  • Own and drive the pipeline and portfolio of clinical, operational, and technological capabilities that advance Cityblock’s care model and operational platform
  • Partner with Clinical, Operations, and Product leaders to identify performance and capability improvement needs, scope them, prioritize them, resource them, and then implement them
  • Deeply understand the capabilities of the Cityblock care delivery model and operating model, and align with clinical and operational leadership to diagnose and identify the most important capability gaps and issues that need to be addressed through innovative new programs that advance and mature our platforms over an accelerated time frame
  • Serve as the steward and process owner for a disciplined innovation management approach, including guiding and driving the organization through an ongoing process of evaluating and prioritizing a portfolio of potential new innovations, internal capabilities needs, investment priorities, risks/benefits, value propositions, adoption enablers and/or barriers, and relevant trade offs
  • Guide teams to develop pilot programs that then graduate into full scale capability innovations that are implemented across Cityblock, including end-to-end project and resource management. Over time, lead optimization of new innovations and capabilities to reach their full potential, and to efficiently support Cityblock’s growth
  • Leverage LEAN and design thinking principles to deliver greater efficiencies and effectiveness. Employ detailed process and workflow analysis with emphasis on member experience, clinical outcomes and quality
  • Partner with leaders across almost every function within Cityblock to drive innovation and serve as a change agent. Partner with vendors, business partners and community organizations for innovations that extend beyond Cityblock’s walls
  • Develop a point of view on the types of innovations that could be game changing, not only for Cityblock but for the broader healthcare industry

You’d be a good fit if:

  • Have 12+ years of experience in healthcare management consulting or similar large-scale initiatives management experience
  • You have experience leading complex organizations to create and pursue a portfolio of capability innovations, with significant program management responsibilities
  • Have general operations experience within health plans, particularly health plans with significant Medicaid, Medicare Advantage and/or duals business lines and advanced value-based care contracting relationships
  • You have experience developing, managing, and optimizing budgets and financial operations related to large implementations
  • You have built a personal brand and reputation for delivering outstanding service to senior executives
  • You have demonstrated an ability to engage, inspire and build credibility and trust with your teams and with clinical executives
  • You consider yourself to be a “guru” or “wonk” on healthcare delivery innovation, technology trends and emerging medical equipment technologies

Nice to have:

  • Similar project/program management experience within a top consulting firm, private equity firm, innovative care delivery organization, or high growth medical group or health system
  • Experience leading innovation in a high growth technology company
  • Established yourself as an industry thought-leader in clinical innovation
  • You have strong familiarity with HIPAA and other applicable health care regulatory frameworks
  • Master's degree in related field (i.e., MBA, MHA, MPH, MS)
