We deliver personalized primary care, behavioral health, and social services through a network of neighborhood hubs with deep community-based partnerships and world-class technology.

We partner with payers and at-risk providers, taking capitated financial risk to care for their most vulnerable, high and rising risk members. Additionally, we invest in strategic partnerships with community-based and social services organizations, in some instances bringing them into the value equation through sub-capitation and performance-based compensation.

Over the next year we’ll grow quickly, including entering new markets, each with their own commercial relationships and field-based teams. This role will be a key contributor to the success of our business.

The role:

In this role, you will be responsible for managing the process of connecting our HARP members with Home and Community Based Services (HCBS). This entails working alongside the Cityblock care team to connect with the member, inform the member of the services they may be eligible for, completing the eligibility assessment, and working alongside the member and the care team to develop the HARP plan of care. At that point, you will be responsible for collaborating with Beacon, Emblem’s contracted behavioral health organization, as well as the HCBS providers. You will ensure that the member is connected to the HCBS provider of their choice, and that the HCBS provider is connected to the member’s care team. Additionally, you will support the hub with general behavioral health services, including evaluations, crisis services/management/intervention, carrying a small panel for therapy as needed/desired, leading group psychotherapy, and participating in behavioral health programming.

You will:

  • Work closely with the care team (especially Community Health Partners and Behavioral Health Specialists) on identifying HARP members who are interested in Home and Community Based Services.
  • Work with care team members to explain the HCBS process to members, and design a process that is most comfortable with the member.
  • Complete the HCBS eligibility assessment independently or with the existing care team depending on the member’s preference, and using the assessment to develop the HARP plan of care alongside the member and care team.
  • Collaborate with Beacon and HCBS providers for members connected to the services, until at least the member’s second visit with the HCBS provider. At this point, the member’s continued support will be provided by the Community Health Partner and the rest of the care team.
  • Be available ongoing to support community health partners and other care team members around HARP and HCBS.
  • Support the Behavioral Health team by assisting with behavioral health evaluations.
  • Support the health hub with behavioral health crisis services, management, and intervention.
  • Care a small panel of members for either short term therapy.
  • Lead group psychotherapy and assist with behavioral health programming.

You’d be a good fit if:

  • You have an LCSW
  • You have experience with a seriously mentally ill population, and other significant behavioral health diagnoses/concerns.
  • You enjoy crisis management and intervention.
  • You have familiarity with Care Management, primary care, and/or behavioral health programs and practices
  • You have the ability to work in a fast-paced, growth oriented environment where independence, flexibility and adaptability are required
  • You have the ability to manage multiple projects
  • You have the desire to contribute to a learning and evolving model of care, and a quickly growing staff
  • You are excited about how technology can support your work and help drive the ongoing evaluation toward new and better care

Nice to have:

  • You have experience with the HARP population and the process of connecting members to HCBS.
  • Experience in a technology-enabled healthcare setting
  • Advanced proficiency in Google Suite
