Who We Are:

At Cityblock Health, our mission is to radically improve the health of urban communities, block by block. We integrate services to address physical, behavioral, and social needs together, delivered at community hubs and through field-based care teams to make healthcare more accessible, supportive and goal-driven for the people who need it most.

Every aspect of the Cityblock care model is carefully designed to focus on our members, engaging and empowering them to own and improve their health through trusted relationships. Through mobile interdisciplinary care teams, we are flexible in how we deliver care, meeting members where they are, and together developing and working longitudinally through a personalized and integrated Member Action Plan (MAP). The care team collaborates to support each member’s whole health and social needs. Enabled by custom-built technology, we build capacity, deliver care and dramatically change members’ opportunities and outcomes.

Partnering with community-based organizations and a well-respected commercial partner in New York, and backed by the top healthcare investors in the country, we are reorganizing the health system to focus on what matters to our members—and leading the move from transactional, fee-for-service medicine towards high-value, relationship-based partnerships.

The role:

As an Account Manager, you will be primarily responsible for the retention and relationship of your assigned health plan partner. You will serve as the primary point of contact, and trusted partner, fostering relationships across different stakeholders within both organizations, in order to advance our shared mission. This includes facilitating various initiatives across multiple domains, such as delegated care management, quality, risk adjustment, ongoing data integration, analytics and reporting.

You will:

  • Develop contacts and relationships throughout complex health plan systems and foster collaboration with key stakeholders to achieve shared objectives
  • Understand Cityblock’s contractual agreements with health plan partners, and collaborate with internal teams to insure that Cityblock upholds its obligations
  • Establish transition plans with Business Development (BD) and Implementation to provide a seamless and professional experience to our partners when transitioning from each phase of their engagement with Cityblock
  • Engage with BD and Implementation teams to be informed of regional, market, or plan-specific issues, and key areas of focus
  • Prepare, conduct, and follow-through on regular meetings with health plan partners, including bi-weekly and monthly operations meetings
  • Contribute to preparing Joint Operating Committee (JOC) materials for internal stakeholders including performance against KPIs, new strategic initiatives, and items requiring decisions or escalation by committee members
  • Support delegated care management lead on complying with NCQA care management programs, including interaction with other 3rd party delegated entities, facilitating data sharing, and coordinating annual audits
  • Facilitate quality and risk adjustment initiatives by implementing and monitoring adherence to established protocols for data sharing
  • Collaborate with Member Management and Finance teams to facilitate timely and accurate monthly reconciliation and new cohort assignment processes, from member selection to empanelment.

You’d be a good fit if:

  • You have 5+ years experience working with a health plan or provider organization
  • You have great attention to detail and project management experience
  • You’re skilled at using KPI dashboards and other data to drive initiatives
  • You are interested in advancing innovative models of care
  • You can effectively engage and collaborate with senior leaders to execute complex initiatives

Nice to have:

  • NCQA delegation experience

You should include these in your application:

  • A resume and/or LinkedIn profile.
  • A 1-2 paragraph response indicating why this job is compelling to you.

Cityblock values diversity as a core tenet of the work we do and populations we serve.
