ESSENTIAL DUTIES AND RESPONSIBILITIES

Coordinate with meals on wheels provider the delivery of meals by maintaining daily contact when they are not able to reach the clients. Provide assistance to Case Managers by being the contact person in addressing day-to-day issues with HMDL clients missing their meals. Assist Case Management staff with monitoring calls as needed. Assist Program Director with administrative tasks.

COMPETENCIES

  • Maintain accurate written records of phone calls related to clients missing their HMDL on a daily basis.
  • Contact clients and emergency contact of clients reported missing their HMDL on a daily basis.
  • Document steps taken regarding clients missing their HMDL including contact with the client and/or emergency contacts in comprehensive notes.
  • Maintain telephone/email contact with HMDL coordinators on a daily basis and report about the outcome of HMDL specialist intervention.
  • Coordinate with case managers steps to be taken if clients that are missing their HMDL are not found during a reasonable amount of time.
  • Maintain a daily written log of clients missing their HMDL and provide the Case Management Supervisor and Program Director with a monthly written report of clients that have missed their HMDL consistently.
  • Assist Case Management staff with monitoring calls as needed.
  • Attend trainings as required including mandatory DFTA trainings.
  • Other duties as required by the Case Management Supervisor and/or Program Director.

EDUCATION and EXPERIENCE

  • Associate's Degree in the social work or human services field with experience in the provision of social services.
  • High School Diploma or GED with at least 2 years of experience in the provision of social services.
  • Bilingual Spanish required.
