Position Overview: 

The SILP Case Planner is responsible for all aspects of case management services for a caseload of youth residing in the SILP Program. The case planner provides concrete services, resources, counseling and is an integrated member of the SILP team which strives to ensure improved outcomes for youth. The SILP Case Planner will provide services while residents are in care and provide aftercare services upon discharge. The SILP Case Planner is responsible for all paperwork required by ACS; OCFS and the courts.

Position Qualifications:

BA/BSW degree from an accredited college and experience working with adolescents (Masters is preferred). Must have at least three years of case work/case planning related experience. Must understand the critical nature of timely and accurate documentation of service delivery and case work practice. Must have knowledge and experience in child development as well as adolescent development. Must be a team player and can work independently.
