 Day Program Plan Coordinator 
 

The Day Program Plan Coordinator (DPC) is responsible in assisting program management to ensure ISP Plans, Staff Action Plans and all corresponding documentation and timelines are met. A Day Program Plan Coordinator is responsible for the development of the individualized and person-centered program plans with the purpose of developing opportunities for self-determination, independence, increased productivity and greater community inclusion. The DPC will develop plans to promote independence, community inclusion, individualization, and productivity while respecting the individual’s personal choice regarding lifestyle decisions and their ability to take informed risks.

Education Required:

High School diploma or equivalent

Specialized training, license, certification, skills: Will be required to complete and demonstrate competency and certification in First Aid, CPR, SCIP-R and BIPP.  Must complete Center-Wide, Adult Services division and program specific orientation.  Training is provided by CFDS.  Some programs may require Medication Administration Certification, NYS Driving License in good standing, Road Test by agency trained personnel and agency vehicle to transport person(s) supported.

Education Preferred:

Bachelor’s degree in a related field   

 Experience Required:

At least 3 years of job related experience, specifically working with people with developmental disabilities or related field.  A minimum of an Associate’s degree or two completed years of college may be considered in lieu of 1 year experience.

Rate of pay is commensurate on experience.

$19.80 - $20.90
