What You’ll Do

Reporting to our Field Operations Lead, Galileo’s Health Advocates are at the heart of our care teams, building trusting and supportive relationships with a group of patients to help them meet their goals. As a Health Advocate you are naturally empathetic and a good listener; you understand what’s going on in the lives of the patients you work with and you invest quality time with every patient getting to know them as a real person. Our Health Advocates continuously collaborate with our team of clinicians and social workers creating tailored care plans and then working closely with each patient, ensuring they are well supported and empowered. Specifically, you will:

  • Be embedded within the community, spending more time in people’s homes and in their neighborhoods than in an office
  • Leverage your local knowledge to find and implement creative solutions to address your patients’ health and social barriers 
  • Combine your natural people skills with helpful processes and tools, ensuring each patient is engaged and well-connected to resources
  • Work alongside an interdisciplinary team of devoted clinicians and social workers

You are responsible, organized, and know the devil is in the details. You understand the importance to both the team and to patients of being reliable, prompt, and trustworthy. You have extensive knowledge of and ties to your local community- figuratively and literally speaking the language. You are up for a challenge and are excited about making a true difference in the lives of community members.

What projects are you most excited about?

I'm really excited to launch our mobile app. Of all the products we're working on, the app will be the most visible to patients. For months we have been working closely with our Clinical Team to translate their decades of experience as Doctors into a digital product that feels just as trustworthy and empathetic as they are.
