Job Description:

The responsibilities of the QM Coordinator are as follows:

  • Investigate home care related allegations (review electronic medical record, conduct staff & patient interviews) and provide recommendations for interventions within 48-72 hours.
  • Respond to MLTC & DOH grievances within allotted timeframe.
  • Manage complaint database and monthly reporting to Upper Management in good faith of Quality Improvement Program.
  • Maintains privacy and confidentiality of clients/patients, visitors, and staff.
  • Serves as a resource to staff for educational needs, providing individual and group training as needed & ensures compliance with agency, regulatory, and reimbursement standards...and performs other related duties as required.

Requirements:

  • Baccalaureate degree preferred
  • Critical thinker, highly motivated self-starter, strong organizational skills, ethical standards and professionalism required.
  • Ability to communicate both in writing and verbally required.
  • Healthcare/home care experience in data and system analysis with proven outcome enhancement required.
  • Experience with HHA eXchange required.
  • Proficiency in Word and Excel required.
