Job Description
Performs complex diversified duties related to the operational needs of the department. Coordinates follow-up care for patients post discharge from the Emergency Department /Inpatient / Outpatient settings. Ensures patients obtain appropriate appointment/ specialty referral based on their care needs.
Job Responsibility
1.Performs complex administrative duties which include, but not limited to: scheduling appointments, answering telephones, generating data metrics, updating office workflows.
2.Responsible for accurately and responsively performing varied administrative tasks and problems of moderate complexity.
3.Solves or escalates operational problems and technical issues to senior team members and management; modifies existing tools or processes.
4.Coordinate follow-up care for patients based on their referral needs (i.e. specialty care, primary care, preventative care, etc.).
5.Utilizes multiple scheduling systems and tools (i.e. EMR, ListApp, Excel, Soarian, etc.) to schedule and manage appointments.
6.Provides exceptional customer experience for patient, family, care team members including service recovery, as needed.
7.Ensures patient is referred to appropriate care provider, based on current insurance plan, patient needs, and diagnosis. Collaborating with insurers, providers and practices, as applicable.
8.Provide referral coordination as a service to employees as requested/if applicable.
9.Collects, organizes, and analyzes data to understand program success and opportunities for improvement, as applicable.
10.Participate in departmental meetings and patient care improvement initiatives, as required.

Job Qualification
• Associates/Technical Degree or equivalent combination of education and related experience.
•3-5 years of administrative experience, required and in a health care setting.
Job Preferences:
- Bilingual: English/Spanish or Albanian Preferred
- Shift times:
- Monday-Friday Coverage from (8AM-8PM)
- 8 AM - 4 PM (with 1-hour lunch) = 7 hours
- 9 AM - 5 PM (with 1-hour lunch) = 7 hours
- 10 AM - 6 PM (with 1-hour lunch) = 7 hours
- 12 PM - 8 PM (with 1-hour lunch) = 7 hours
- 9 AM - 5 PM 4 Days Mon-Friday + 2 Saturdays per month = 7 hours
