Pearl Health helps PCPs who are overwhelmed by the demands of an aging population and the unpredictable business model inherent to Traditional Medicare. Over time, this solution will empower PCPs to provide better care to their entire patient population. We believe there is a need to bring humanism back to healthcare by giving physicians the ability to better allocate their time with patients, increasing their autonomy, creating incentives that are aligned with achieving better outcomes through clinical decision-making powered by actionable intelligence, and making risk more available and more readily optimized through an intelligent marketplace. The Center for Medicare and Medicaid (CMS) is launching a new value-based care program to disrupt Traditional Medicare, and we believe that this represents an opportunity to achieve the meaningful change we envision.

Who you are . . .

We are seeking a stellar Performance Optimization Manager, experienced in engaging with physician practices and health systems in a value-based care context and eager to help our current customer base succeed in the ACO REACH model.  This role demands a solid foundational understanding of the healthcare delivery system for primary care practices, administrative and clinical workflow, and the possession of exceptional change management skills. Your mission as the Performance Optimization Manager will be to identify key clinical drivers of performance, inform on key strategies and performance activation plans, and develop scalable and relevant workflow best practices to empower clinical transformation that helps reduce total cost of care and improve quality outcomes for patients.  

You will partner with Customer Success relationship managers, Data Science, and Performance Operations, and Clinical Strategy teams to develop scalable strategies based on performance analysis of practices. Leveraging strong practice workflow and operations discovery tactics, you will deploy empathy and curiosity to catalog and diagnose the current state of value based care activities at the practice level. You will inform and design actionable practice transformation plans in response to discovery and performance analysis data points, and consult with Customer Success relationship managers in implementing targeted and achievable performance improvement initiatives.
Beginning with a focus on our enterprise customer base, you will play a key role in the development and execution of scalable optimization tactics and initiatives to support a wide range of customers including but not limited to: development and iteration on workflow best practice enablement resources, scalable performance improvement plan tooling, and performance improvement tactics.

While this will be a remote position, in-person engagement with provider practices will be critical and some travel is required (up to 30%).

To be successful in this role you will: 

  • Work alongside Customer Success relationship managers to proactively understand and document the value-based care processes for each practice. Be able to translate data insights for practices, and collaboratively suggest actionable measures for improvement.
  • Collaborate with customers in person and virtually to engage in deep discovery, support redesign efforts, support care teams, introduce population health management and Pearl technology, and support achievement towards improved financial, process, and clinical outcomes
  • Consult with Customer Success relationship managers in implementing recommended achievable performance improvement initiatives 
  • Work cross functionally to develop a scalable strategic system that aligns client performance frameworks with targeted short and long-term objectives for product, incorporating market trends, provider expectations and business priorities across a wide array of client geographies, clinical models, and tiers.
  • Utilize this system to identify cost drivers and recommend interventions at the practice level to improve clinical quality and operational efficiency. Work in partnership with Customer success relationship managers and Preferred Providers to develop optimal referral, care management, and admission avoidance strategies, contributing to practice performance.
  • Demonstrate aptitude for relationship building and creating an exceptional experience for our providers.
  • Deeply partner with Customer Success relationship managers to support education and deployment of technology tools that drive practice transformation, and integration of technology tools into practice workflows 

Who you are…

  • Bachelor’s degree or equivalent work experience
  • 3-6 years of experience in clinical transformation, improving clinical quality, or other relevant provider practice-facing field. Experience at an ACO, or tech enablement company is a plus.
  • Experience in interpreting and utilizing clinical data analytics, outcomes measurements, and designing internal dashboarding to drive change and resource allocation
  • Demonstrated experience implementing clinical practice transformation initiatives designed to help provider groups achieve delivery transformation goals (experience implementing in a hospital system preferred
  • Experience preparing and presenting information to clinical and executive level leadership
  • Willingness to travel for face to face meetings Enterprise or health system level provider management experience preferred 
  • A passion for improving and optimizing relationships
  • Excited about helping physicians and impacting Medicare patient outcomes

We are a remote-first company, with a concentration of team members at our NYC headquarters and in the Boston area.  

Compensation and Benefits

The salary range Pearl Health expects to pay for this position is between $110,000-$140,000 per year.  Full time employees are also eligible for annual discretionary bonus. Where a given candidate falls within that range will depend on a variety of factors, including, but not limited to, the candidate’s relevant skills, experience and location, labor market conditions and participation, if any, in other compensation arrangements.  Pearl Health provides its employees a competitive benefit package - for more information please review our benefits page.
