Assess referred patients for hospice admission, educate patients and caregivers, generate and respond to referrals, coordinate with physicians and the interdisciplinary team, develop initial care plans, perform outreach to referral sources, and travel to patient locations including weekends.

Assess, plan, coordinate, implement, and evaluate patient care across the continuum with focus on utilization review, discharge planning, risk management, and referrals. Facilitate community and hospital referrals, collaborate with medical staff and clinical disciplines, document care, and support quality and performance improvement activities.

The Vice President of Quality and Safety oversees quality management and patient safety programs, ensuring compliance with regulatory standards in hospice care.
