Conduct street and door-to-door outreach, distribute HIV/AIDS education and condoms, refer and escort clients to health and support services, document outreach activities, and participate in training and community events to meet program targets.

The Sr. Advanced Practitioner Nurse/Clinician leads clinical affairs, supervises clinicians, and manages medical operations in a CHN Center, focusing on underserved communities.

Provide direct patient care in community health centers: administer medications and injections, perform intakes and venipuncture, assist providers with procedures, coordinate referrals and follow-up, provide health counseling, maintain documentation and nursing competence, and support clinic operations across sites.
