Develop employer relationships and transitional employment opportunities, match and prepare job seekers for interviews, conduct outreach and work readiness training, coordinate referrals and hiring events, track placements and produce performance reports to place 10–15 individuals monthly in unsubsidized employment.

Conduct audits and statistical surveillance of workforce activities to ensure compliance with policies and contract requirements. Implement and maintain continuous quality improvement processes, evaluate reports and survey data, document case management activities, ensure reviews meet State guidelines, and deliver monitoring reports to the QA Manager.

Conduct psychological evaluations and mental health screenings to determine Functional Capacity Outcomes and service track assignments. Provide emergent risk assessments and safety recommendations. Document all interactions in SEAMS, collaborate with medical staff, participate in team meetings and trainings, and ensure evaluations are provided in participants' primary language when possible.
