Provides direct peer support to individuals with mental health conditions using lived recovery experience. Responsibilities include outreach, engagement and retention, case management, recovery education, transition support, treatment team participation, documentation, and collaboration with participants and families. The role requires maintaining confidentiality, using an electronic health record, supporting person-centered care, and helping participants pursue education, employment, and recovery goals.
