Teams at Little Flower Children and Family Services of New York

Provides direct support to foster care families and youth, helping coordinate parenting time, treatment plans, meetings, court proceedings, school and IEP conferences, medical consents, and reunification goals. Maintains case documentation in the Connections database, completes admission and incident records, supports case planners, participates in home assessments and discharge planning, and follows child welfare policies and procedures.

Supports the Clinical Director in managing youth mental health services, including clinical supervision, therapy, assessments, documentation, referrals, treatment planning, compliance, audits, and coordination with families and multidisciplinary providers. Provides at least seven weekly clinical service hours, oversees clinical staff performance and documentation, supports trauma-informed programming, represents the mental health department in treatment meetings, and coordinates clinical internships.

Supports up to 50 middle school students and families through educational advocacy, academic monitoring, school selection, IEP and special education support, tutoring referrals, extracurricular connections, and school stability planning. The specialist collaborates with parents, foster families, case planners, schools, tutors, mental health providers, and community organizations; tracks academic data, prepares reports, coordinates services, and advocates for appropriate educational supports.
