MonmouthCares

MonmouthCares is a nonprofit Care Management Organization serving Monmouth County, New Jersey. It provides face-to-face care management and comprehensive service planning for youth and families with complex emotional, behavioral, intellectual/developmental, substance-use, or physical-health needs. Using the Wraparound model, the organization coordinates community services, develops individualized plans, provides advocacy and family support, and monitors care so children can thrive in their communities.
