The Clinical Case Manager II supports high-needs individuals facing complex challenges due to serious mental health issues and is involved in the criminal justice system by providing case management, coordinating care, and collaborating with various services to promote recovery and compliance with court mandates.

The Case Manager will assess client needs, build relationships, provide support, and facilitate access to resources for program participants in the Queens Supervised Release Program.

The Peer Specialist provides peer support and advocacy to individuals in the Supervised Release Program, focusing on recovery, engagement, and personal growth while handling a caseload of 15-20 participants with high needs related to mental health and substance use.
