Provide supportive case management for chronically homeless clients in scatter-site housing: conduct mental health and psycho-social assessments, deliver counseling, crisis intervention, relapse prevention, facilitate support groups, develop service plans, document client contacts, coordinate life-skills training and community referrals to support housing retention.

Manage a caseload of RWA program clients, coordinate referrals for mental health, substance recovery and benefits, document progress in databases, facilitate groups, provide crisis intervention, and liaise with DHS and outside agencies to support employment, housing, and program goals.

Provide supportive case management for residents in permanent supportive housing: maintain caseloads and records, conduct assessments, develop and implement service plans, link residents to healthcare and benefits, provide counseling/crisis intervention, liaise with community providers, facilitate groups, monitor rent/budgeting and unit conditions, and complete required reports and trainings.
