Under the guidance of the Unit Nurse Manager, The Clinical Coordinator is responsible for coordinating the clinical practice and care to residents in a Long Term Care and Short Term Rehab setting. This role is a liaison between residents, families, medical providers and Loretto employees. Conducts patient assessments, implements and evaluates the effectiveness of nursing care delivered to the residents.
Salary: $84,323 - $94,702 Annually
Schedule: Monday - Friday- 08:00am-4:30 PM
ResponsibilitiesResponsibilities include:
- Oversee the delivery of Quality Patient Care
- Compliance with NYSDOH and CMS regulations and facility policies and protocols
- Orients, trains and mentors new employees
- Provides health training to patients and their families
- Assists in gathering and analyzing data for QAPI
- Acts as primary liaison with medical providers
- 1+ year experience in nursing preferred
- Current RN NYS license
- Established patient assessment skills
- Knowledge of best-practice standards
- Skilled to respond to emergent situations
- Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes
- Exceptional interpersonal and customer service skills
- Proficiency in speaking, listening, reading and writing the English Language is required
