Duties includes, but are not limited to:

1.    Search and retain volunteer site for consumers that attend Day Habilitation Program regulated under OPWDD.

2.    Monitor consumers and staff at their assigned worksites.

3.    Create safe, supportive environment in which Day Habilitation consumers can learn skills necessary to function more independently.

4.    Ensure that consumer’s Habilitation Services are carried out on a daily basis. (like goals, go to volunteer site, recreation of Friday activities)

5.    Monitor the completion of all habilitation documentation in contemporaneous.

6.    When applicable, coach and council consumers regarding job performance improvement.

7.    Document consumer worksite attendance on a daily basis.

8.    Develop and implement consumer’s monthly schedule.

9.    Worksite follow-up and supervision:

10.    Promote community integration.

11.    Assist the Day Habilitation Coordinator in supervising staff, as assigned.

12.    Promote consumer progress with regard to independent living, life skills, personal hygiene, work readiness, positive self-image, and appropriate demeanor.

13.    Assist supervisor in coordinate monthly staff meeting.

14.    Monitor physical plant for cleanliness and safety.  (ex. kitchen, bathroom, laundry room, vent openings, computer room, TV room ADL room and refrigerator)

15.    Coordinate with program coordination to identify staff needs, staff training etc.

16.    Oversees the maintenance, safety and cleanliness of the site assigned vehicle.

17.    Develop a schedule and ensure that fire drills are conducted twice a month and the necessary documentation is completed.

18.    Plan and coordinate client recreational schedule.

19.    Develop and monitor staff in regard to:

•    Completion of habilitation documentation
•    Driving schedule
•    Orientation of new staff including (test driving)
•    Addressing and supervising staff regarding routing issue and/or consumer related issue
•    Routing schedule
•    Plot driving direction for staff to follow when driving

20.    All other tasks as required or assigned by your Supervisor or Director of Department.
 

Qualifications

A High School Diploma required.  College Degree preferred.

Valid New York State Driver’s License, required with at least 3 years driving experience.

Prior Supervisory experience preferred.

Knowledge of the consumer population.

Ability to be flexible to the needs of the program.
