Reporting Relationships: Employment Specialist/Case Manager report to Assistant Program Director.

Principal Duties and Responsibilities:

Under the general supervision of the Assistant Program Director, Employment Specialists/Case Manager are expected to perform the following principal duties and responsibilities.


  • Participates on the initial screening and assessment team to determine whether tenant is suitable for independent living and appropriate to be housed in the housing program.
  • Develops and update support plans, case notes and other documents related to a tenants file and in accordance with funder requirements.
  • Ensures compliance with organizational standards of confidentiality in handling of records.
  • Completes vocational assessments with tenants; explores and identifies interests, employment history, experience and skills.
  • Develops job leads by establishing contacts with private and public sector employers or through employment agencies and maintains on-going communication with employers.
  • Guides tenants in exploring and matching work or vocational experiences to their interests and career streams, and delivers work-readiness seminars.
  • Maintains updated information and progress notes in tenant's case records on client’s employment progress.
  • Tracks and reports outcomes and provide data updates on tenants as directed.
  • Provides direct counseling and on-going assessments, as needed.
  • Represents the agency to the community at large and participates in professional meetings.
  • Complete thorough, timely and accurate documentation consistent with AAPCI policy, and contractual requirements.
  • Advocate and connect tenants to medical, mental health, and other supportive counseling services.
  • Regularly coordinate with all collateral contacts, including but not limited to family, providers, Workforce 1 Career Centers, HRA and NYS DOL etc.
  • Engage with community partners to provide services onsite and offsite.
  • Conduct visits in the tenant’s unit to ensure compliance with lease and program rules.
  • Effectively guide tenants through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, HRA, SNAP, and other services as needed.
  • Manages an assigned caseload of tenants ensuring comprehensive case management services are provided to both families and single adults.
  • Performs other related duties as may be requested by immediate supervisor.
Qualifications

Minimal Qualifications:

Degree Requirement: Bachelor’s degree in a related field (social work, mental health counseling, psychology, sociology etc.) with at least 3-5 years’ experience in a residential setting.


Other Qualifications:

  • Experience working with one (1) or more populations including: mental health, substance abuse programs, inpatient/outpatient treatment, homeless and supportive housing populations.
  • Attend external and internal meetings and trainings including social services, and all staff meetings as scheduled.
  • Strong verbal and interpersonal communication skills with a focus on providing excellent services.
  • Demonstrated ability to interact effectively and collaboratively with a diverse community of tenants, program staff, and other departments.
  • Strong time management and organization skills.
  • Ensures exceptional attention to accuracy and quality of work.

Compensation and Benefits

Base salary range and benefits information for this position are being included in accordance with requirements of New York State and City pay transparency laws.  Please note that salaries may vary for different individuals in the same role based on several factors, including but not limited to location of the role, individuals’ competencies, education/professional certifications, qualifications/experience and performance in the role. 

Compensation

The base salary compensation range being offered for this role is $42,000 per year.
