About the Role:

In this role, you will primarily provide administrative support to our providers to help ensure they can provide the most effective and efficient care possible to our members . Our members often face economic and social challenges in addition to having mental and physical health needs.  It is integral to our care model that we meet people where they are, both physically and emotionally. You will be part of an interdisciplinary care team, focusing on clinical visits taking place in a virtual setting. 

This position will report to the Medical Director and is a key member of the Cityblock care team and plays an integral role in defining and delivering a state-of-the-art experience to Cityblock members.  


Requirements for the Role: 

  • You have 3+ years of experience working in a clinical operations setting
  • You are minimally certified as a medical assistant 
  • You are highly efficient at synthesizing and summarizing medical information across multiple data sources 
  • You have a commitment to detail and follow-up, ensuring there is clear resolution or next steps for any member or provider need   
  • You are comfortable managing ambiguity, proactively identifying systems/process breakdowns and proposing solutions 
  • You are comfortable using a variety of computer systems, and have familiarity with electronic health record systems
  • You are eager to be part of an interdisciplinary team, including physicians, EMTs, nurses, social workers, community health partners.
  • You enjoy being a team player who jumps in wherever necessary
  • You have the ability to establish and maintain effective working relationships with members and other care team members
  • You have excellent communication skills, including the ability to effectively interact with all members of the care team, and with members of the public
  • You are compassionate, open-minded, and non-judgmental


How We Define Success:

Administrative Support (~90%)

  • Work across internal and external systems to procure medical records and upload into the EMR; coordinate with external providers to manually obtain records if necessary 
  • Prepare charts for visits, including synthesizing key information across medical records, the EMR, and our care facilitation systems (Commons); this will specifically include reconciling internal and external data pertaining to medical history, medication, allergies, labs/imaging and any gaps in care
  • Support providers with visit documentation as needed to ensure all information is accurately reflected in both the EMR and Cityblock’s care facilitation system (Commons) 
  • Coordinate post visit follow-up care; this includes but is not limited to obtaining additional medical records, helping members navigate health systems to complete lab tests, imaging, or external appointments with specialty providers
  • Facilitate member hand-offs to longitudinal care teams

Direct Member Care (~10%)

  • Virtually room and interview members at the start of the appointment and record medical history in chart
  • If needed, assist the primary care provider during the exam over video


Nice to Have, But Not Required:

  • Experience working collaboratively with an interdisciplinary care team
  • Experience in a primary care setting
  • Multi-lingual preferred
