The registered nurse is responsible for applying clinical nursing knowledge and skills to promote the safety and comfort of patients and families according to legal, organizational, and professional standards. The RN serves as a single point of contact for patients, their families, caregivers, and referring physicians to assist in accessing clinical and supportive services across the care continuum. This RN provides guidance to patients and families to improve their understanding and promote their participation with Palliative Care. This position will facilitate patient appointments, including diagnostics, specialty services, and consults, as well as offer educational and emotional support.
Primary Responsibilities:
  • 30% Clinical Activities
    • Participate in patient care activities and adheres to requirement of a Registered Nurse when providing direct patient care both in the clinic and in the home. This is demonstrated by:
    • Ability to make sound nursing judgment based on nursing theory
    • Develops and maintains collaborative relationship with a multidisciplinary team and contribute to the plan of care, including patient emergencies
    • Prioritizes urgency of patients' needs and is a patient advocate and encompasses the continuum of illness including physical, psychosocial, emotional, and spiritual needs of the patient
    • Performs in-person and telephone patient assessment and documentation via electronic medical record (EMR) with documentation that is legible, concise, accurate, and timely
    • Adheres to scope of practice and policy to support and educate patients and families on symptoms and treatments through the end of life discussions, regarding advanced directives and informed consent
    • Provides patients with verbal and written educational materials and promotes emotional support to patients related to the disease process, prescribed medication, and/or symptom management
    • Accurately administers medication to Palliative Care patients as prescribed by the physician
    • Ability to organize and utilize equipment and supplies according to patient needs and resources based upon the physician order
    • Ability to consider needs of the department by preplanning and proactively adjusting as demonstrated by reviewing the EPIC daily schedule, rooming patients and travel with Physician as required for schedule home visits.
    • Accurately completes daily and monthly clinical exam room logs
    • Assumes responsibility for competency of practice and takes initiative for professional development and continuing education
    • Communicates all important facts and patient complaints to the nurse manager and or director
  • 10% Quality Improvement Activities:
    • Actively participates in quality improvement activities, as evidenced by:
    • Understands the Quality Improvement Process
    • Identifies opportunity for quality improvement and makes appropriate suggestions to nursing management
    • Participates in quality improvement projects as appropriate
    • Completes assigned quality improvement tasks in a timely and accurate manner
  • 15% Customer Service:
    • Maintains active professional relationships with physicians, co-workers, external customers, patients, and caregivers by assessing needs and levels of satisfaction. Demonstrates a positive, friendly, and courteous manner. Identifies opportunities to enhance customer service, as evidenced by:
    • Positive patient and physician satisfaction
    • Reports inefficiencies, deficiencies, and unusual occurrences to manager and director
    • Serves as a role model to other staff members
    • Maintains confidentiality on matters to which they are privileged

  • 10% Employee Management:
    • Demonstrates ability to oversee coordination of patient services, as evidenced by:
    • Assumes responsibility of assigned clinic patients
    • Ability to explain policies, procedures, and methods to others
    • Ability to appropriately delegate tasks to unlicensed assistive personnel
    • Assists in the orientation and training of new employees
  • 5% Resource and Inventory Management:
    • Ensures proper use of resources (time and supplies), as evidenced by:
    • Carefully uses supplies and medicines to avoid waste
    • Reports when supplies are low and need ordering
    • Keeps accurate time records
    • Stocks clinic with supplies as needed
    • Assists in keeping supply closet neat and organized
    • Identifies opportunities to be more cost-effective and efficient
  • 5% Regulatory Compliance:
    • Complies with local, state, and federal rules and regulations and accreditation guidelines, as evidenced by:
    • 100% of regulatory concerns or unusual occurrences are reported to the manager and or director
    • When patient injury or disaster occurs the Manager and or Director is notified immediately
  • 5% Safety:
    • Adheres to all facility safety regulations and guidelines related to patient safety. Maintains unit in safe condition, as evidenced by:
    • Keeping hallways clear
    • Securing electrical wiring
    • Reporting broken equipment
    • Participating in quarterly safety compliance drills and/or in-services
    • Identifies, corrects, and reports unsafe conditions immediately
  • 20% Infection Control:
    • Provides an environment to prevent and control the spread of disease within the clinical setting. Which includes but is not limited to:
    • Hand washing
    • Aseptic and sterile technique

Required Qualifications:
  • Graduate from an approved professional registered nursing program
  • 1+ years clinical experience. May consider an LVN-RN transition, with LVN 4+ years of experience
  • Current and valid Texas RN license
  • Current BLS issued by the American Heart Association
  • Proven ability to use equipment and related supplies
  • EPIC experience oxygen administration, and intravenous therapy
  • Experience with EMRs
  • Basic computer skills

Preferred Qualifications:
  • Bachelor's degree in nursing
  • Ambulatory Care or Hospital or Home Health experience
  • Specialty nursing certification in Palliative one appropriate to the specialty area assigned
  • Proven windows bases computer skills for selected patient population and for CPR

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29 - $52 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
