Job Description

The Bureau of Equitable Health System is the healthcare systems bureau of Department of Health & Mental Hygiene. Our mission is to apply policy, evidence, and practical expertise to improve equity in health care delivery at the individual, organizational, and systems levels. We do this by engaging primary care providers and other healthcare organizations to implement evidence-based strategies; leveraging information to support planning and technical assistance for providers and payers; advancing policy to close the racial equity gap for priority health outcomes; and surfacing opportunities where health care can influence and connect consumers to social support and addressing the whole person, beyond physical ailments.
Duties will include but not be limited to:
Applied Research, Analysis, and Evaluation:
- Perform applied research and analytic activities related to primary care delivery, population health, and health equity initiatives.
- Develop evaluation plans, research questions, and performance measures to assess implementation processes and outcomes.
- Review, analyze, and interpret quantitative and qualitative data; evaluate findings; and formulate evidence-informed recommendations.
- Maintain records and prepare written reports, summaries, and presentations to communicate findings to leadership, funders, and stakeholders.
Project Coordination and Implementation Support:
- Plan and coordinate assigned practice transformation and technical assistance projects with medical practices, community health centers, hospitals, and community-based organizations.
- Develop and maintain project workplans, timelines, and documentation; monitor progress; and identify implementation issues and risks.
- Coordinate project activities with internal program staff and external partners to ensure timely completion of deliverables and alignment with related initiatives.
- Assist in coordinating research and evaluation efforts across BEHS programs and other DOHMH units.
Practice Transformation and Technical Assistance:
- Manage a small portfolio of Patient-Centered Medical Home (PCMH) and related practice transformation initiatives.
- Provide consultative and technical assistance support to participating practices related to care delivery redesign, care coordination workflows, and population health interventions.
- Contribute to the development and refinement of practice transformation curricula, tools, and guidance materials.
Supervision, Training, and Knowledge Sharing:
- Provide technical guidance and mentoring to facilitators and project staff engaged in research, evaluation, and practice transformation activities.

- Liaise with internal and external stakeholders to support coordinated implementation efforts.
Additional Duties:
- Assist with the preparation of grant and funding application materials, including project descriptions and evaluation components.
- Perform related duties as assigned.
PREFERRED SKILLS:
A minimum of three (3) years of experience performing applied research, evaluation, quality improvement, or practice transformation work in healthcare or public health settings.
Demonstrated experience in planning, coordinating, and evaluating projects involving healthcare providers or community-based organizations.
Strong analytic, writing, and presentation skills.
Ability to exercise independent judgment while working collaboratively in a matrixed environment.
Knowledge of:
Primary care delivery systems and healthcare operations
Electronic Health Records (EHRs) and health IT platforms
Population health management and quality improvement
Social Determinants of Health

Qualifications

1. For Assignment Level I (only physical, biological and environmental sciences and public health) A master's degree from an accredited college or university with a specialization in an appropriate field of physical, biological or environmental science or in public health.
To be appointed to Assignment Level II and above, candidates must have:
1. A doctorate degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and one year of full-time experience in a responsible supervisory, administrative or research capacity in the appropriate field of specialization; or
2. A master's degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and three years of responsible full-time research experience in the appropriate field of specialization; or
3. Education and/or experience which is equivalent to "1" or "2" above. However, all candidates must have at least a master's degree in an appropriate field of specialization and at least two years of experience described in "2" above. Two years as a City Research Scientist Level I can be substituted for the experience required in "1" and "2" above.
