Our flagship Ready, Willing & Able (RWA) program provides paid transitional work experience, transitional housing, advanced occupational training, and comprehensive supportive services to individuals experiencing homelessness. The Doe Fund is also one of New York City’s largest nonprofit developers of permanent, affordable, and supportive housing, with more than 1,900 units in operation or pre-development. The organization has an annual operating budget of $80 million and a full-time staff of nearly 500, a significant portion of whom are graduates of our programs.

The Doe Fund is currently seeking a high-energy, self-motivated, experienced Senior Case Manager to join our team. The successful candidate will have experience working with formerly incarcerated individuals, Mental Health Organizations, Homeless Services, and/or Housing organizations. The Senior Case Manager will manage a caseload and be a team player, assisting the Associate Director and Director with management-related tasks. This position will be based out of our 510 Gates Avenue Program in Brooklyn.

Responsibilities:

  • Manage a caseload of challenging clients with high service needs and assist other Case Managers and the management team with providing services.
  • Generate Service Plans/Independent Living Plans with and for clients.
  • Monitor the progress of clients toward meeting goals established in their Independent Living Plans and the RWA Program.
  • Research and coordinate access to outside referral sources for Mental Health, Substance Recovery, Benefits, Entitlements, and Primary Care services.
  • Communicate with DHS and other agencies as needed.
  • Collaborate with clients; assist in goal setting and self-development; work as a team with other staff to achieve program-wide participation, employment, housing, graduation, retention, and other program goals.
  • Organize and facilitate caseload and other groups on various topics.
  • Maintain current, accurate, complete, and timely documentation of trainee progress in charts and case-noting databases (TDF’s Client Tracking Database and DHS’s CARES).
  • Provide crisis intervention to clients in order to resolve any issues or concerns that may arise.
  • Assist with generating reports related to program services.
  • Perform other duties as assigned.

Qualifications:

  • An Associate Degree is required. Bachelor’s degree with a major in Psychology, Sociology, Counseling, Social Work, or other Human Services concentration preferred. 
  • At least 3+ years of experience in Human Services/Case Management
  • Experience working with challenging high-service needs populations preferred
  • CASAC a plus
  • Strong interpersonal and written/verbal communication skills are required
  • Working knowledge of MS, Word, Excel, PowerPoint, and Google
  • Bilingual preferred.
