Pharmacy1st Health is building the clinical infrastructure for rural America by transforming independent pharmacies into integrated care delivery hubs. Our model combines primary care, pharmacy services, and chronic disease management under one roof, creating a new, lower-cost site of care for underserved communities.

We operate across multiple states and are rapidly expanding a network of rural clinics co-located with pharmacies. Our goal is to deliver scalable, high-quality care while improving access, outcomes, and cost efficiency.

Role Overview

The VP, Clinical Services, will lead the design, development, and deployment of new clinical programs across Pharmacy1st’s clinic network.

This is not a purely administrative role. You will be responsible for translating clinical insight into scalable service lines that drive both patient outcomes and sustainable revenue. You will work at the intersection of care delivery, operations, and product, building programs that can be deployed consistently across rural markets and deliver measurable impact.

Key Responsibilities

Clinical Program Development
  • Design and launch new clinical service lines across primary care, urgent care, and chronic disease management

  • Identify high-impact opportunities based on patient needs, payer incentives, and operational feasibility

  • Develop standardised care protocols, workflows, and clinical pathways

  • Ensure programs are optimised for rural populations with limited access to care

Care Model Innovation
  • Build and refine team-based care models leveraging physicians, nurse practitioners, pharmacists, and care coordinators

  • Develop and implement Collaborative Practice Agreements (CPAs) to extend clinical capacity

  • Integrate pharmacy and medical services into a unified patient experience

Execution and Scaling
  • Pilot new services in select markets and scale successful programs across the network

  • Define KPIs for each clinical service line, including utilisation, outcomes, and financial performance

  • Partner with operations teams to ensure consistent execution across sites

Quality and Outcomes
  • Establish clinical quality standards aligned with CMS, payer programs, and evidence-based guidelines

  • Monitor outcomes, patient safety, and adherence to protocols

  • Drive improvements in metrics such as HEDIS measures and Medicare Star Ratings

Cross-Functional Leadership
  • Collaborate with product and engineering teams to build workflows into the EHR

  • Work with finance and leadership to ensure services are economically viable

  • Support payer strategy by aligning services with reimbursement opportunities (RHC, value-based care, etc.)

Provider Leadership
  • Serve as a clinical leader and mentor for physicians, nurse practitioners, and pharmacists

  • Support recruitment, onboarding, and ongoing training of clinical staff

  • Help build a strong, mission-driven clinical culture

Qualifications

Required

  • MD/DO or Nurse Practitioner (NP) with an active U.S. license

  • 5+ years of clinical experience in primary care, urgent care, or ambulatory settings

  • Demonstrated leadership experience in a clinical or healthcare delivery organisation

  • Experience designing or implementing clinical programs or care models

  • Strong understanding of U.S. healthcare reimbursement, including Medicare and value-based care

  • Will consider remote for the right candidate.

Preferred
  • Experience working in rural or underserved communities

  • Familiarity with pharmacy integration or team-based care models

  • Experience with RHCs, FQHCs, or similar settings

  • Track record of scaling clinical operations across multiple sites
