Position Summary:  Manage a comprehensive program of services including individual and group assessment, counseling and linkage with health, educational, entitlement and vocational programs to ensure consumers successful and expeditious placement into the community.  This position is responsible for ensuring the provision of all casework services in accordance with the Act team.  This position will have unsupervised contact and conversational interaction with consumers.

 Essential Accountabilities
  1. Ability to screen clients, conduct assessments and determine areas of need.
  2. Coordinate case management services in order to ensure comprehensive development of an individual’s service plan. 
  3. Facilitate in obtaining entitlements for consumers.
  4. Act as a clinical liaison.
  5. Control the quality of services through regular formal supervision, case conferences and case consultation to ensure a high standard of services.
  6. Establish and implement ongoing casework policies and procedures to ensure quality of service.
  7. Develop treatment plans with other members of the ACT team and the consumers.
  8. Manage the timely collection and analysis of pertinent client and program data to generate reports, evaluate program performances and determine still unmet needs.
  9. Coordinate services with all other members the ACT Team.
  10. Facilitation of groups for consumers.
  11. Ensure all documentations are completed in a timely fashion and remain in compliance with agency & OMH regulations.
  12. Responsible for ensuring physical plant is in compliance with agency and OMH regulations at all times.
  13. Participate in OMH approved ACT Orientations and Trainings.
  14. Ability to operate an agency vehicle.
  15. Attend any scheduled training’s relevant to the position or deemed necessary by Team Leader.
  16. Participate in activities of other staff members in their absence or during periods of staff shortage.
  17. Assists clients with case management needs; transportation to and from appointments, applying and following up with all eligible benefits and entitlements and any other needs deemed appropriate by treatment team.
  18. Maintain confidentiality at all times.
  19. Represent the Program at meetings and training’s not otherwise specified.
  20. This does not limit the assignment of duties or exclude the performance of other duties not mentioned as assigned by supervisor
  21. Responsible for reporting and documenting all incidents to immediate supervisor within 24 hours.
  22. Assists in the planning, developing and implementation of goals and training to further develop participants’ skills, including but not limited to:           
  1. Budgeting
  2. Grooming
  3. Nutritional planning
  4. Use of public transportation
  5. Use of community resources
  6. Shopping
  7. Housekeeping
  8. Physical health & development
  9. Adaptive behavior/independent living
  10. Nutrition
  11. Vocational development
  12. Social development

     22. Acts as a role model for participants; furthers participants in researching and obtaining community resources appropriate and applicable to their needs and goals; follows-up on any aftercare/recommendations for treatment.

  1. Provide crisis intervention and mental health counseling as needed.
  2. Input all visits and data into Vellohealth software application.

  1. Demonstrate ability to: maintain required written documentations; communicate relevant information to other staff and supervisor(s), verbally and in writing.
  2. Ability to work flexible schedules as work schedules, trainings and location are subject to change
  3. Participate in activities of other staff members in their absence or during periods of staff shortage.
  4. This does not limit the assignment of duties or exclude the performance of other duties not mentioned as assigned by supervisor, the Team Leader, Deputy Director, Director, and/or Senior Management.
  5. Provide 24-hour/seven day a week emergency coverage via emergency cell phone to provide crisis intervention services after program hours on a rotating basis, if applicable.
  6. Ability to drive agency vehicle.
  7. Attend all required trainings and any deemed relevant by the position's supervisor.

QualificationsQualifications: 
  • High school diploma or GED required, Bachelors in human services preferred. Minimum of 2 years’ experience working with individuals with mental illness required.
  • Two years experience working with Mental Health and Substance Abuse consumers preferred.
  • Experience working with behavioral health population preferred.
  • Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record.
  • Valid New York State driver’s license required.  Vehicle required for LI teams only– candidates are subject to regulatory background checks and must be willing to utilize own vehicle for home visits.
