Provides direct psychiatric services through an Assertive Community Treatment team, including psychiatric evaluations, medication assessment and prescribing, dose adjustments, client education, injectable medication administration, laboratory and toxicology orders, documentation, crisis intervention, and community-based visits for individuals with serious mental health conditions, co-occurring substance use disorders, and homelessness.

Provides community-based mental health nursing services within an ACT team. Supports medication administration, injection scheduling, prescription coordination, pharmacy communication, medication record updates, toxicology documentation, nursing assessments, progress notes, and consumer wellness groups. The role primarily serves individuals with serious mental illness or MICA needs, requiring crisis intervention skills, clinical experience, teamwork, and extensive community-based work.

Provides Individual Placement and Support employment services to ACT consumers, including job development, rapid placement, job coaching, counseling, skills training, treatment planning, case documentation, employer networking, and ongoing post-placement support. Collaborates with a multidisciplinary ACT team, develops community partnerships, connects consumers with resources, and maintains records in NYESS, the agency EHR, and regulatory systems.

Provides Individual Placement and Support employment services to ACT consumers, including job development, rapid placement, job coaching, counseling, skills acquisition, and ongoing workplace support. Conducts community employer outreach, participates in multidisciplinary assessment and treatment planning, maintains case records and employment data, develops support linkages, and enters information into NYESS and agency systems.

Provides nursing and medication-support services to adults with serious mental illness in an Assertive Community Treatment setting. Assists with injections, oral medication preparation, medication records, refills, pharmacy coordination, nursing assessments, progress notes, toxicology forms, and weekly health education groups. Works primarily in the community, collaborating with registered nurses, prescribers, pharmacy staff, and the clinical team.

Provides on-site administrative and reception support for an ACT mental health team. Responsibilities include data entry, phone coverage, clinical information updates, billing and Medicaid tracking, insurance eligibility verification, managed-care communications, petty cash and card reconciliation, electronic check requests, missing-client searches, and vehicle-log maintenance.

Provides direct behavioral health services to children, adolescents, adults, and families, including diagnostic evaluations, risk assessments, crisis intervention, treatment planning, psychotherapy, group services, and care coordination. Maintains clinical documentation, collaborates with internal and external providers, and participates in mobile crisis response with rotating after-hours on-call coverage.

Oversees residential mental health programs, including clinical, operational, financial, regulatory, and building functions. Supervises Program Directors and per diem staff, ensures compliance with New York regulations, prepares sites for external reviews, manages incidents and grievances, supports admissions and discharges, assists with budgeting and purchasing, coordinates services, supports hiring and training, and serves as an interim Program Director when needed.

Directs clinical, operational, financial, and facilities functions for a mental health residential program. Supervises staff, oversees resident care and case records, ensures regulatory compliance, manages crises, hiring, training, quality assurance, finances, rental collections, facility maintenance, licensing, audits, and resident grievances. Maintains relationships with providers and referral sources while ensuring adherence to organizational policies and applicable housing, health, safety, and mental health regulations.

Directs clinical, residential, operational, and financial functions for a mental health residence. Oversees case management, resident care plans, records, staffing, supervision, crisis response, regulatory compliance, audits, quality assurance, finances, occupancy, facilities maintenance, vendors, training, and licensing. Ensures adherence to mental health, housing, safety, and health regulations while managing resident grievances, admissions, discharges, incidents, and relationships with referral sources and providers.
