Teams at Postgraduate Center for Mental Health

Provides direct psychiatric services through an Assertive Community Treatment team, including psychiatric evaluations, medication assessment and prescribing, dose adjustments, client education, injectable medication administration, laboratory and toxicology orders, documentation, crisis intervention, and community-based visits for individuals with serious mental health conditions, co-occurring substance use disorders, and homelessness.

Provides community-based mental health nursing services within an ACT team. Supports medication administration, injection scheduling, prescription coordination, pharmacy communication, medication record updates, toxicology documentation, nursing assessments, progress notes, and consumer wellness groups. The role primarily serves individuals with serious mental illness or MICA needs, requiring crisis intervention skills, clinical experience, teamwork, and extensive community-based work.

Provides Individual Placement and Support employment services to ACT consumers, including job development, rapid placement, job coaching, counseling, skills training, treatment planning, case documentation, employer networking, and ongoing post-placement support. Collaborates with a multidisciplinary ACT team, develops community partnerships, connects consumers with resources, and maintains records in NYESS, the agency EHR, and regulatory systems.
