Provide peer-based support to residents by modeling recovery, teaching coping and life skills, facilitating groups, advocating for participants, monitoring progress, and assisting with community reintegration under the Residential Services Team.

Manage day-to-day operations of the case management office: handle high-volume calls, order supplies/equipment, maintain patient discharge logs and files, process invoices, manage ADP/time for CM staff, input appeals/denials data, ensure department cleanliness, and perform Discharge Facilitator duties when absent. Work schedule 8AM-4PM.

Provide case management and housing-related services to residential clients; perform assessments, develop quarterly collaborative service plans, and maintain regular client contact. Complete electronic charting, progress notes, and reporting in the program database per confidentiality and hospital policies. Promote client integration into the residence and community, monitor capacity for independent living, and support outreach to meet program goals.
