Clinical Practice: Applies the nursing process to care deliveryTime: 0%

ASSESSMENT: Performs an initial and ongoing assessment which encompasses the physical, psychosocial, cultural, spiritual and educational needs of the patient and their caregiver(s). Reviews and analyzes available data considering medical, nursing and patient priorities.
PLAN OF CARE: Using evidence and best practice standards, contributes to the development of an individualized plan of care in collaboration with the patient, family and multidisciplinary team.
MPLEMENTATION: Applies knowledge and clinical skills to provide competent level of direct patient care and performs/assists with procedures. Adheres to policies, procedures and established standards of care in delivery of nursing services while creating a safe and therapeutic environment for patients, caregiver(s) and self.
EVALUATION: Evaluates progress toward expected outcomes and modifies individualized treatment plan. Identifies patterns of patient response and contributes to changes in practice standards.
PATIENT EDUCATION: Assesses patient and caregiver learning needs and provides education about the disease, treatment plan, actual and potential side effects of treatment and medications, and self care measures. Participates in patient and caregiver education programs. Identifies and participates in the resolution of potential and actual barriers to learning and adherence with treatment plan.
PSYCHOSOCIAL SUPPORT: Provides care that is patient-centered. Collaborates with the multidisciplinary team to develop a plan for meeting the needs of patients and caregivers experiencing distress.

• Conduct thorough assessments of patients with wounds, ostomies, and continence issues in accordance with evidence-based practices and clinical guidelines.
• Develop and implement individualized care plans that address patient needs, preferences, and clinical evidence.
• Perform advanced wound care procedures, including dressing changes, use of specialized wound care products, and monitoring for signs of infection or complications.
• Provide care for patients with complex wounds, including those associated with cancer diagnoses.
• Deliver comprehensive ostomy care, including fitting ostomy appliances, educating patients on ostomy management, and addressing common concerns such as skin irritation or leakage.
• Assess and manage continence issues, focusing on patient-centered approaches and providing tailored education and support for individuals experiencing urinary or fecal incontinence.
• Assess, plan, and implement plan of care for patients with fistulas to improve comfort and quality of life.
Education and Training:
• Educate patients and their families about wound, ostomy, and continence care.
• Provide training and support to healthcare staff on best practices in wound, ostomy, and continence care.
• Develop and deliver educational programs and materials for patients and staff.
Consultation and Collaboration:
• Serve as a consultant for healthcare teams regarding wound, ostomy, and continence care.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
• Participate in interdisciplinary team meetings to discuss patient care plans and outcomes.
