Position Summary:

The Reentry Mentor is a core member of the Fedcap IMPACT team, responsible for targeted outreach, engagement, and participant support for individuals being released from Rikers Island Department of Correction (DOC) facilities or NYS Correctional Facilities. This role serves as the first point of contact for individuals transitioning from jail or prison, helping guide them into services and maintaining consistent follow-up to promote their ongoing engagement. The Reentry Mentor supports communication, scheduling, and service connection pre-release, and may accompany individuals to community providers post release.

Participant Engagement and Outreach

  • Facilitate orientations in housing units to market program services to Persons in Custody (PICs)

  • Collaborate with Department of Corrections staff and Fedcap Transition Specialists for assignment of units to market to PICs due for release on a weekly basis

  • Follow up with participants to provide updates on requested housing referrals, documentation needs, or to deliver court letters and other supportive materials in a timely manner.

  • Conduct outreach and engagement 2x a week to assigned units inside Rikers and to participants in community within 30 days of release date or with a scheduled court date.

  • Maintain 80% timely documentation of all sessions, outreach, and follow-up activity in FedcapCARES and DOC-tracking systems.

Collaboration & Team Support

  • Participate in weekly team supervision and monthly partnership meetings to discuss progress, performance, and participant needs.

  • Collaborate with Transition Managers, Workshop Facilitators, and community partners to ensure seamless coordination for 80% of active cases.

  • Contribute to monthly data reporting and quality assurance reviews, identifying areas of improvement and ensuring less than 10% error rate in documentation.

Data Accuracy & Documentation

  • Enter all encounter notes, and outcomes into FedcapCARES within 48 hours of service delivery.

  • Maintain at least 80% data accuracy in participant records and documentation.

  • Ensure all data entry, reports, and case notes comply with DOC, MOCJ and Fedcap confidentiality and compliance

Professional Conduct & Compliance

  • Maintain DOC clearance and comply with all facility and Fedcap policies regarding safety, conduct, and confidentiality.

  • Complete 100% of required DOC and Fedcap training courses annually and maintain compliance certifications.

  • Demonstrate punctuality and professional conduct in 90% of attendance

  • Represent Fedcap with professionalism, cultural sensitivity, and a commitment to equity and inclusion

You're a Great Fit for This Role If You Have:

  • A High School diploma or equivalent required; Associate degree or equivalent experience preferred.

  • Minimum of 1 year of experience in outreach, peer navigation, case support, or community engagement. 

  • Lived experience of incarceration or justice involvement strongly preferred. It must be more than 2 years from incarceration to be cleared for Rikers access.

  • Experience working with justice-impacted individuals, public assistance recipients, and those affected by poverty. 

  • Strong interpersonal and communication skills with the ability to build trust quickly

  • Comfortable working within jail-based settings and engaging with diverse populations

  • Must pass NYC Department of Corrections security clearance to work on Rikers Island 

Compensation: 

$19.24-$20.68 per hour
