JOB SCOPE:
This is a direct service position requiring specific knowledge of mental health, and skills in crisis intervention and stabilization. As part of the ACT you must be willing to go out into the community to provide services. This position requires clinical experience, and the desire to work as part of a team.
ESSENTIAL FUNCTIONS:
· Participates in daily morning meeting.
· Assists RN with administering IM's and preparing oral medications for weekly delivery by the clinical team.
· Assists RN with maintaining monthly IM calendar.
· Coordinates with Team prescribers and pharmacy to ensure consumers receive medications and refills.
· Transcribe orders and updates Medication Administration Records.
· Assists with logging in medication deliveries and completing toxicology request forms.
· Assists with completion of annual nursing assessments and daily progress notes.
· Conducts weekly Healthy Living group with consumers.
· Performs other duties, consistent with the goals and objectives of the program, as directed by the Team Leader.
QualificationsEDUCATION AND EXPERIENCE:
· Licensed Registered Nurse in New York in conjunction with either an Associate’s, Bachelor’s or Master’s degree.
· Experience serving the mentally ill/mentally ill chemically addicted (MICA) population.
WORK ENVIRONMENT:
This position operates 80 percent of the time in the community, rotating in an office setting as needed and for team meetings.
